Community Health Worker (Transportation Focus) - Primary Care Anywhere (Field-based Role)

Job Summary

Working within an interdisciplinary care team, the Community Health Worker is responsible for proactively engaging patients identified as high-risk and implementing targeted interventions to address transportation needs and increase access to care.

This role requires an understanding of how socioeconomic stressors can impact ability to engage in healthcare and subsequent health outcomes. Experience will ideally include prior work with patients with behavioral health diagnoses, as well as in navigating local community-based resources and benefit applications.

This role has a mobile presence, involving travel to patients' homes, treatment facilities and community-based settings, and assigned care teams to facilitate and foster connections.

Major Duties and Responsibilities

  • Develop a holistic view of Social Determinants of Health related specifically to patient transportation needs

  • Identify existing barriers related to transportation and leverage available resources and support.

  • Support care planning and subsequent action steps related to transportation related needs, coordinating plan of care with interdisciplinary team

  • Serve as liaison between the patient and direct care providers, assisting in navigating both internal and external systems (i.e. treatment centers and specialists)

  • Refer patient to necessary services and supports related to transportation and navigation of application for benefits including, Medicaid, HCBS, working to reduce costs associated with transportation

  • Perform all other duties and responsibilities as required

Organizational Responsibilities

  • Participate in interdisciplinary review of and coordination around complex patients

  • Maintain patient confidentiality in accordance with HIPAA

  • Document patient encounters in medical record system in a timely manner

  • Follow general policies related to fire safety, infection control and attendance

Required Qualifications

  • High School Diploma or equivalent

  • Community Health Worker certification, or willingness to complete within one year

  • Minimum of 2 years of experience working in human services and navigating community-based resources, especially around transportation

Preferred Qualifications

  • Bachelor's Degree in applicable discipline

  • Familiarity with state Medicaid guidelines and application processes

  • Experience working with patients with behavioral health conditions and substance use disorders

  • Prior experience conducting home visits and knowledge of field safety practices

  • Training as an LPN or LVN

  • Training as an EMT or paramedic

  • Bilingual in English/Spanish with the ability to speak, read and write in both languages without limitations and assistance

Working Conditions

This role has a mobile presence, involving travel to patients' homes, treatment facilities and community-based settings, and assigned clinics to facilitate connections.

Workstyle: Combination in clinic and field, local travel to meet with patients

Location: Greater Atlanta including Cherokee, Clayton, Cobb, Coweta, DeKalb, Douglas, Fayette, Forsyth, Fulton, Gwinnett, Hall, Henry, Jackson, Jasper, Newton, Paulding, Rockdale, Spalding, Walton Counties

Hours: Must be able to work a 40-hour work week, Monday through Friday 8:00 AM to 5:00 PM, over-time may be requested to meet business needs.

Tuberculosis (TB) screening : This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Driver's License, Reliable Transportation, Insurance This role is part of Humana's Driver safety program and therefore requires an individual to have:

  • Required to have a valid driver's license.

  • Position will be subjected to motor vehicle record monitoring.

  • Required to maintain auto insurance with the limits below per our policy:

Associates are expected to maintain personal vehicle liability insurance. Associates must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.$47,700 - $65,600 per yearThis job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
