Apply empowerment-based prevention and intervention strategies including motivational interviewing to improve community health and facilitate screening, follow-up, and treatment for alcohol use.Evaluate patients for additional cancer-related risk factors and provide referrals to meet those needs. Ensure timely receipt of screening, results, and scheduling for follow-up, as needed.Facilitate patients requiring additional testing or treatment through patient navigation, including addressing barriers patients may experience. Address personal and environmental issues with the patient to maximize emotional, social, and physical well-being and the appropriate use of healthcare resources.Provide training for providers and staff on new screening criteria and techniques for unhealthy alcohol use to increase referrals for decreasing unhealthy behaviors and/or receiving treatment for alcohol use disorder.Work closely with the Program Coordinator, Manager, and Program Director of the Department of Medicine at the University of Texas Dell Medical School to ensure coordinated efforts and effective program implementation. Partner with clinic providers and support staff to assess patient needs and deliver comprehensive healthcare services, both in-person at the clinic and through telemedicine platforms. Develop and maintain strong internal relationships and partnerships with colleagues, including clinical managers, support staff, providers, and business office personnel, to foster a collaborative and supportive work environment.Ensure all tasks provided and associated with patient care, including documentation of patient contacts and progress, patient administrative processes, and related duties comply with all regulatory and accreditation standards. Attend seminars and maintain all licensure and/or certification requirements to support ongoing education and adherence to best practices.Perform other related job functions as assigned.
