Job Description
High Lakes Health Care, of Praxis Health, is seeking a dedicated and compassionateCommunity Health Workerto support individuals in accessing healthcare services, promoting healthy behaviors, and addressing social determinants of health. Working as part of an integrated care team, the CHW provides case management and patient support in both clinic and community settings, helping to reduce barriers to care and improve health outcomes. As a key liaison between patients, the healthcare system, and social services, the CHW assists individuals in navigating available resources and ensuring continuity of care. The ideal candidate is flexible, self-directed, and skilled at problem-solving in dynamic situations, proactively seeking the information needed to assist patients effectively.

Essential Job Functions:
Establish trusting relationships with individuals and communities to provide culturally appropriate health education and support.
Assist individuals in navigating healthcare and social service systems, including scheduling appointments, accessing transportation, and understanding insurance options.
Conduct outreach efforts to identify and connect with underserved populations, including rural and marginalized communities.
Provide one-on-one or group health education on topics such as chronic disease management, nutrition, mental health, and preventive care.
Advocate for clients by helping them access resources such as food assistance, housing support, and employment services.
Work collaboratively with healthcare teams to develop care plans and track patient progress.
Document interactions and maintain accurate records in compliance with state and organizational guidelines.
Participate in training and professional development to stay informed about best practices and emerging health concerns.

Minimum Qualifications
High school diploma or equivalent required; additional education in public health, social work, or a related field is a plus.
Completion of a Community Health Worker training program is required.
Strong communication and interpersonal skills with the ability to build rapport and trust.
Knowledge of local healthcare systems, social services, and community resources in Oregon.
Ability to work independently and as part of a multidisciplinary team.

General Physical Requirements
Manual dexterity for office machine operation including computer and calculator; stooping, bending to handle files and supplies, mobility to complete errands for deliveries, or sitting for extended periods of time.
The employee must regularly lift and /or move up to 10 pounds, frequently lift and/or move up to 25 pounds and occasionally lift and/or move up to 50 pounds.
Stress can be triggered by multiple staff demands and deadlines.
Work is performed in an office environment. Involves frequent contact with staff and patients.
