The Diagnosis and OASIS Specialist is responsible for coding pertinent medical information on patient classifications according to coding guidelines, OASIS Item Guidance and CMSdirectives. This person will also be responsible for performing OASIS reviews to ensure OASIS accuracy and for assuring data integrity through quarterly reviews. This position will work collaboratively with professional staff to assure timely and appropriate flow of information.
JOB DUTIES AND RESPONSIBILITIES:
Reviews documentation from Liaison/Intake nurse, referral source, Medical Director and admitting clinician. Utilizing standard coding conventions makes recommendations related to diagnoses codes, sequencing and the plan of care.
Evaluates responses to OASIS assessments to ensure compliance with CMS guidance and congruency within the document.
Communicates with admitting clinician either verbally or through written methodology to validate audit findings and recommend additional and/or more specific documentation.
Works closely with clinicians and physicians to ensure documentation of all medical conditions to accurately reflect the patient’s clinical status and care needs.
Maintains accuracy rate of coding data as measured through quarterly quality reviews.
Consistently meets established productivity targets for coding assignment and record review.
Communicates and collaborates with Quality Managers, Patient Care Managers and Directors as needed.
Acts as a resource to staff regarding documentation and coding including education and participation in identified projects as directed by Quality/Resource Utilization Manager.
Follows efficient and effective mechanisms to communicate opportunities for improvement to staff and trends to Managers and Directors.
PHYSICAL AND SENSORY REQUIREMENTS:
Exertional activity - Occasionally lift and/or carry 50 lbs. Frequently lift and/or carry 25 lbs. Being able to frequently lift or carry objects weighing up to 25 pounds is often more critical that being able to lift up to 50 lbs at a time. Stand and/or walk at least 6 hours in an 8-hour work day. Very few medium occupations in the national economy are performed primarily in a sitting position.
Nonexertional activity - Stooping (bending at the waist) and crouching (stooping and bending at the knee) required frequently in most medium jobs. Handling (grasping) – required frequently in most medium jobs. Fingering (fine manipulation) – required only occasionally in most medium jobs
Important: The functional capacity to perform medium work includes the functional capacity to perform sedentary and light work.
Physical Demands - Good physical and mental health. Neat, clean, free from body odors, conforms to the uniform code. Visual and auditory acuity required to provide job functions. Physical stamina for walking, standing for long periods, turning, stooping, bending, climbing, stretching and lifting in the provision of job responsibilities. Finger and hand dexterity necessary to handle office.
Environmental Conditions - Inside – Office environment. Outside - Traveling to office errands in extreme cold and heat, wet and/or humid conditions, snow, ice and/or fog. Hazards - road and driving.
EDUCATION:
PA-Licensed Registered Nurse or Physical Therapist. A bachelor’s degree in specific discipline preferred.
TRAINING AND EXPERIENCE:
Two years Home Health experience within the past five years required. Experience and/or knowledge of Hospice care preferred. HCS-D preferred or obtained within two years. OASIS certification preferred or obtained within one year.
