Description
Licensed Practical Nurse - Day Hab
30 Hours/Week
Monday - Friday, 8:00 a.m. to 2:30 p.m.
Wage: $22.61-$26.39
The Licensed Practical Nurse (LPN) is responsible for the provision of basic nursing services, i.e. medication administration, first aid and health observation, and reporting of health and safety concerns relating to program individuals. This position’s specific duties and responsibilities include, but are not limited to:
Provide first aid and general nursing care; make nursing observation along with documenting and reporting of injury and/or illness of program individuals to the assigned Register Nurse and any required family notification
Observe program individuals, identify any changes in medical condition or complaints of by individuals then report such symptoms or changes to a physician or assigned Register Nurse to assist with diagnosis and treatment planning
Be familiar with program individuals medical information and personality to obtain information useful in diagnosis and treatment
Accompany individuals on medical appointments or to hospital/emergency room as requested and provided supervision when no other staff is available
Administer medications as prescribed in accordance within NYS Scope of Practice, OPWDD regulations and agency policy
Maintain, monitor and review medical and health information on assigned individuals to ensure that all health and medical concerns are being addressed at admission and as required in accordance with policy and regulation along with keeping assigned Register Nurse informed of changes
Perform or review medication inspections at designated sites
Complete and/or review documentation pertaining to nursing services of assigned program individuals and ensure all needed items and/or signatures are included
Provide contact for health related issues and physicians (within Scope of Practice) with family and/or caregivers as needed
Consult with other program nursing staff on health/safety issues; may also cover, or be accessible for other program sites as requested
Participate in individual planning conferences for medical and/or health related concerns
Conform to program changes and regulations and keep abreast of developments in the field
Perform required duties in a productive and safe manner
SPECIAL REQUIREMENTS:
Possess good interpersonal, written, oral, and communication abilities and effective time management/organizational skills
Be computer literate with knowledge in agency operating systems and applicable software required for department needs.
Demonstrate sound judgment in normal work and emergency situations
Must physically be able to perform required certifications and lift fifty (50) pounds

Requirements
Must be licensed and currently registered as a Licensed Practical Nurse by the New York State Department of Education. Experience working with intellectually/developmentally disabled is preferred.
A valid driver’s license and good driving record acceptable to the Agency’s insurance carrier.
