Trinity Health Clinical Dietitian Coordinator for Bariatric Surgery Full Time Days in Boise, Idaho

Position Purpose:

Saint Alphonsus Health System is hiring a Metabolic and Bariatric Surgery Care Coordinator in Boise, Idaho to coordinate bariatric patient care with providers, triage bariatric patients (telephonically/in-person), conduct pre-visit chart reviews to assess and anticipate bariatric patient care needs, develop and provide patient education, coordinate care across the continuum, and support quality improvement activities within the practice at Saint Alphonsus Medical Group Metabolic and Bariatric Center.

In addition, this role will be responsible for coordinating the accreditation within the Saint Alphonsus Regional Medical Center (SARMC) and will matrix report to SARMC leadership. The Care Coordinator works collaboratively with facilities, providers and staff to provide bariatric care coordination across the health care continuum for all bariatric patients.

  • Schedule Information: This position is scheduled for 40 hours a week Monday – Friday in Boise, Idaho. Weekends and call are not required. The work location in the Mulvaney Building on the Boise Saint Alphonsus campus.

Minimum Qualifications:

  • Licensed in the state of Idaho as a Health Care Professional (ie RN/Dietician/PA/PT).

  • Licensed in the state of Oregon as a Health Care Professional (ie RN/Dietician/PA/PT) within 6 months of hire.

  • Bachelor's of Science in certified field preferred.

  • Registration as an MBS Coordinator preferred.

  • Three years of direct patient care experience desirable.

What You Will Do:

  • Works in coordination with all MBS team members including PSRs, MAs and bariatric insurance coordinator to provides high quality, compassionate and seamless care for our bariatric patients.

  • Assists/develops written protocols and education of nurses detailing the rapid communication and basic response to critical vital signs that is specifically required to minimize delays in the diagnosis and treatment of serious adverse events for the metabolic and bariatric population.

  • Manages internal bariatric patient data base for all bariatric patients in their preoperative and postoperative journey.

  • Leads and coordinates internal MBS team meetings.

  • Coordinates care across the care continuum for the metabolic and bariatric program including:

  • Serves as liaison with patients and their families, providers and other clinical staff.

  • Acts as a liaison with hospitalized patients and the clinic including follow up with patients by phone shortly after hospital discharge.

  • Provides clinical care and interventions based on the specific clinical program requirements supported:

  • Utilizes effective clinical process (assessment, diagnosis, planning, implementation, and evaluation) to coordinate patient care for the specific program supported.

  • Monitors Medical Assistant activities including patient screening/rooming, medical records retrieval, medication refill process, and overall patient throughput throughout the clinic and via electronic means (phone, fax, etc.), as well as specific clinic tasks assignment per the specific program requirements.

  • Huddles with providers and other clinical staff throughout the day to review patient care concerns and assists patients in following plan of care.

  • Conducts chart reviews and screening of all bariatric patients including:

  • Determining the stability of the patients from a clinical, psychiatric and social support structure/living situation standpoint prior to entering the program.

  • Identifying existing patients overdue for visits, labs, or referrals and arranges for follow-up services as appropriate, and initiates our contact protocol to ensure patients are made aware of their need for ongoing care and follow up.

  • Works in collaboration and communication with service delivery partners regarding patient status and missing records.

  • Reviews patient's pertinent medical history, current diagnosis, smoking, alcohol and recreational drug use history, provider's plan of care, and allied health consultations to assist in implementing action plans.

  • Coordinate’s patient care, referrals and communication with multidisciplinary team members (dieticians, psychiatrists, psychologist, diabetes care management team and primary care providers.

  • Organizes and lead bariatric support group meetings.

  • Organizes outreach with referring providers as needed.

  • Develops experience and detailed knowledge of the MBSAQIP accreditation standards for ongoing compliance, patient safety, data collection, quality improvement efforts, and education related to the various aspects of metabolic and bariatric surgery.

  • Works in close relationship with the MBS Director and MBS clinical reviewer, reviewing MBS outcomes data, identifying opportunities for quality improvement.

  • Serves as a liaison between the center and the MBSAQIP.

  • Helps organize and lead the MBS committee meetings along-side the MBS Director.

  • Helps refine, maintain and implements clinical pathways and protocols, patient education, monitoring outcomes data and collection, quality improvement initiatives, and the education of hospital staff with a focus on metabolic and bariatric patient safety.

  • Communicates with relevant hospital leadership and all bariatric proceduralists regarding pathway changes, quality reporting, and quality improvement initiatives.
