Matrix Providers Clinical Pharmacist in Bremerton, Washington

Matrix Providers is hiring a Clinical Pharmacist to join our team of talented professionals who provide health care services to our Military Service Members and their families in at Naval Hospital Bremerton/ Bremerton, WA. Employment Status: Full Time Compensation: TBD Schedule: The HCWs shall provide services Sunday through Saturday, including the day of observance of Federal holidays between the hours of 0700 and 2100. The HCW shall not be scheduled more than a 12.75-hour shift (including a 45-minute uncompensated meal break) in a 24-hour period. The HCWs shall be scheduled no more than 4 holiday shifts. The HCW shall not exceed 80 hours of service in a two-week period. Benefits: Competitive financial package with a comprehensive insurance package including health, dental, vision, and life coverage. o Accrued Paid Time Off (PTO) o Paid Holidays (Outlined in Handbook) o 401(k) Plan The Clinical Pharmacist must have and meet the following: Degree/Education: Doctor of Pharmacy (Pharm.D.) / Graduate from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE). Completion of a clinical pharmacy residency or fellowship accredited by the American Society of Health System Pharmacists or American College of Clinical Pharmacy or documentation of appropriate education, training, and/or CME in the practice of clinical pharmacy. Experience: As required to meet clinical competency requirements specified in the Service-specific credentialing instructions. Licensure/Registration: Current, full, active, and unrestricted license to practice as a pharmacist. Certification: Basic Life Support for Healthcare Providers, American Heart Association, or American Red Cross Healthcare Provider Course. Certifications in addition to Basic Life Support: Current, full, active, and unrestricted license to practice as a pharmacist. Clinical Pharmacist Core Job Duties: Assess patients response to drug therapy and planning drug therapy based on physician-established diagnoses. Order and evaluate laboratory tests necessary to evaluate drug therapy effects and outcomes. Initiate, modify, or discontinue medications for ongoing therapy of chronic disease states (e.g., hypertension, hyperlipidemia, anticoagulant, diabetes, asthma, refill clinics, etc.), in cooperation with the medical staff. Monitor and manage pharmacotherapy requiring periodic adjustment due to specific or changing pharmacokinetic characteristics (e.g., aminoglycosides, phenytoin, antithrombotic). Administer prescription or nonprescription drugs according to established agreements or MTF protocols. Assess metabolic needs and order therapeutic enteral or parenteral nutrition products in the inpatient setting. Evaluate medical and medication histories for drug-related problems and adjusting drug therapy accordingly. Consult with other healthcare providers (e.g., physicians, dietitians, nurses, physical therapists, etc.) about patient treatment needs or options. Perform a full range of pharmacist procedures. Review patient profiles at the time of dispensing in order to closely monitor medication therapy. Ensure prescription directions for use, dose, medication interactions, therapeutic duplications or overlaps, allergy information, age specific dosage, and other pertinent data are appropriate for individual patients.
