Description

The Health Education & Access Coordinator works as part of a school-based interdisciplinary team to help students and families with personal and environmental difficulties, which may predispose illness, or interfere with obtaining maximum benefits from medical care. Ensures all adolescents are connected to relevant benefit programs.

Responsibilities:

  • Conducts individualized health education during SBHC appointments.

  • Makes referrals to medical provider and social work for all new patients.

  • Maintains chart for each encounter; clinical care supervised by collaborating clinician.

  • Conducts follow-up with patients as medically indicated.

  • Assesses individual and community needs for health education on an ongoing basis.

  • Works with Community HealthCorps Health Educator to develop/select/adapt and implement a health curriculum to be delivered to students in the classroom and small group setting.

  • Evaluates effectiveness of curriculum and revise as needed with implementation of current best practices.

  • Screens children and families for Medicaid, CHP, FHP, FPBP, PCAP, and WIC eligibility.

  • Completes the Access NY application for eligible families.

  • Provides intensive tracking and follow-up with applicants to ensure that all required supporting documentation is obtained.

  • Works with Outreach Coordinator to implement insurance-related outreach strategies.

Qualifications

  • HS diploma or GEDrequired

  • Bachelors degree preferred

  • Computer literacy with proficiency in MS Word, Excel, and PowerPoint.

  • Demonstrated organizational, interpersonal, oral and written communication skills and the ability to handle multiple assignments at any time.

  • 1-2 years experience in health education.

  • Demonstrated familiarity counseling adolescents on repro health (experience preferred).

  • Demonstrated familiarity with health insurance issues.

  • Able to commit to at least 2 years in the position.

  • Ability to generate, interpret and analyze data from multiple sources.

  • Bi-lingual, Spanish preferred.
