Responsibilities Job Description
We are seeking a casual physical therapist to join our rehab team at M Health Fairview Ridges Hospital in Burnsville!
In collaboration with patients and families, the physical therapist is responsible for providing safe and effective delivery of patient care within scope of practice. This includes examination, evaluation, diagnosis, planning, intervention and establishing outcomes.
FTE: .01, casual position.
Schedule: One weekend per month (Saturday and Sunday) for 8-hour shifts.
Opportunities to pick up additional weekday and weekend shifts as available!
M Health Fairview Rehabilitation offers a broad range of services that serve patients across 10 acute-care hospitals, 3 post-acute settings and 70 outpatient adult and pediatric therapy clinics. Consisting of Physical, Occupational and Speech Therapy as well as Audiology and Cardiac & Pulmonary Rehab, our therapists collaborate with colleagues in all medical settings and offer dozens of specialty programs.

Rehab’s continuing education program offers 40+ continuing education courses per year at no cost to employees and further supports numerous external CE courses each year.

The physical therapist partners with patientd and families while performing evaluation and re-evaluation.
Develop an individualized plan of care through coordination
Capture and apply pertinent data, performs examination, and resolves treatment diagnosis
Analyze data to identify needs and potential risks, settles prognosis for recovery, and establishes individualized goals
Implement and modify care plan based on reassessment and patient response
Coordinate care and communicates effectively with interdisciplinary team for all aspects care & establishes appropriate discharge plan
Partner with patients and families to provide skilled intervention
Implement and alter treatment to reflect change in patient status and response to treatment
Assess patient and/or caregiver learning needs and provide appropriate education
Documentation/Billing
Use acceptable medical terminology and follows professional practice standards
Complete documentation in a timely manner per department standards
Stay in sync with department billing standards
Professional Expectations
Prioritize tasks and patient care for effective time management
Understand and follow professional licensure/certification requirements
Advise team members and/or supervise students as designated
Pursue professional development to support clinical standard processes
Quality Improvement
Craft and implement improvements, and participate in and support entity/system initiatives
Meet or exceed department standards for productivity
Required
Physical Therapy registration/licensure in the state of Minnesota or temporary license in MN
Current Basic Life Support (BLS) certification
Preferred
Doctorate degree in Physical Therapy
1+ year of physical therapy experience
