Texas State Job Bank Case Manager in El Paso, Texas
Experience/Education:
Requires a Bachelor's Degree from an accredited college or university with a minimum number of hours that are equivalent to a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education, or early childhood intervention; or possession of any equivalent combination of education, training, and experience which provides the requisite knowledge, skills, and abilities. Experience with trauma-informed services; cognitive behavioral therapies, including DBT; and motivational therapies (i.e., motivational interviewing).
Language: English required (Spanish is a plus) to communicate with clients and staff

Driver's License:
Must possess and maintain a valid state Driver's License with an acceptable driving record.
Requirements:
Knowledge of trauma-informed theories, principles, and practices (including multi-faceted understanding of concepts such as community trauma, intergenerational and historical trauma, parallel processes, and universal precautions), preferred.
Must possess and maintain Qualified Mental Health Professional-Community Service (QMHP-CS) certification.
Must be able to pass TB, criminal background and drug screen.
Experience in delivering high-quality, client-centered care
Able to document and navigate Electronic Health Record (EHR) independently
Ability to maintain strict client confidentiality
Ability to effectively communicate and interact with individuals involved in all aspects of programming - children, parents, school faculty and staff, grantors, funders, partner agencies, members of the community and associates
Serves as a representative of (DSBH), displaying courtesy, tact, consideration and discretion in all interactions with associates, patients school system staff and faculty, grantors, funders, regulators and members of the public
Proficient computer skills, including working knowledge of Microsoft Office Suite
Ability to work effectively on a multidisciplinary team
Language Skills: Ability to read, analyze and interpret general and professional documents. Ability to write routine reports and correspondence. Ability to effectively present information in one on one and small group situations
Ability to foster teamwork; train associates and colleagues
Excellent organizational skills, including the ability to work productively
Staffing requirements, including criteria that staff have diverse disciplinary backgrounds, have necessary State required license and accreditation, and are culturally and linguistically trained to serve the needs of the clinic's patient population.
Credentialed, certified, and licensed professionals with adequate training in person-centered, family-centered, trauma informed, culturally-competent and recovery-oriented care
Physical Demands:
Performs sedentary work that involves walking or standing some of the time and involves exerting up to 15 pounds of force on a regular and recurring basis or sustained keyboard operations.

Duties:The functions listed below are those that represent most of the time spent working in this position. Management may assign additional functions related to the type of work of the position as necessary.
Establishes rapport with individuals in communities that are participating in treatment
Provides on-going case management services: Routine Case Management, Intensive Case Management, Medication Training and Support, Skill Training and Development, Psychosocial Rehabilitation Services and/or Crisis Intervention for eligible individuals.
Case management services also include general symptom management, community living skills (resources), social skill development, housing, and employment related skills to increase independence and ability to maintain community tenure.
Coordinates and provides related services.
Demonstrates a positive role model to youth and/or adults and advocates on their behalf.
Attend meetings to report/staff on the individuals status.
Responsible for coordination, planning and implementation of the individuals treatment plan
