Herkimer County Senior Community Health Nurse in Herkimer, New York
DISTINGUISHING FEATURES OF THE CLASS: This position involves the performance of high level community health nursing activities including responsibility for assessment of health needs and developing the plan of care for individuals and families. The position is distinguished from that of Community Health Nurse by the assignment of more difficult cases and increased responsibility for making patient assessments and amending care plans, along with the evaluation of data and planning of policies and evidence based interventions. The incumbent is also responsible for the initial implementation of nursing care plans and providing nursing care as well as the periodic re-evaluation of individual and family nursing needs. The work is performed under the general direction of a Supervising Community Health Nurse with wide leeway allowed for the exercise of independent judgment for planning and carrying out assignments. Supervision is exercised over Community Health Nurses and other subordinate staff. Does related work as required. TYPICAL WORK ACTIVITIES: -Gives skilled nursing care and prescribed treatments to patients in their homes and clinics and demonstrates nursing care to patients and families; -Leads and participates with a team providing nursing care and evaluates the effectiveness of team activities; -Leads and participates in the duties such as pediatric nurse or family nurse practitioner; -Provide for the continuity of patient care by promoting referral of hospital and clinic patients to appropriate community agencies; -Coordinates plans for service with nutritionists, social workers, physical therapists, physicians and other professional workers concerned with individual and family health care; -Counsels and guides individuals and families towards self-help in recognition and solution of physical, emotional and environmental health problems; -Provides nursing services in clinics and schools; -Teaches classes, addresses groups and participates in community planning related to nursing and health; -Participates in the conduct of surveys and studies related to health matters; -Evaluates and complies data, records, reports and statistical information for evaluation and planning of evidence based community interventions and programs; -Participates in and responds to various public health emergencies FULL, PERFORMANCE, KNOWLEDGES, SKILLS, ABILITIES AND PERSONAL CHARACTERISTICS: Thorough knowledge of current community health nursing practice; thorough knowledge of the administrative organization of community facilities; skill in the application of current nursing procedures and techniques of patient care; ability to plan and coordinate nursing care for individuals, families and groups and supervise nursing personnel assigned to assist with this care; ability to communicate effectively; ability to establish and maintain cooperative working relationships; ability to accept and utilize guidance; ability to perform duties in accordance with ANA Code for Professional Nurses; acceptable physical and mental health to carry out the responsibilities of the position.
MINIMUM QUALIFICATIONS: Either (A) A baccalaureate degree in nursing from a regionally accredited or New York State registered four-year college or university and one year experience as a Registered Professional Nurse; Or (B) A registered professional nurse with: (a) two years experience working as a Registered Professional Nurse. SPECIAL REQUIREMENT: Licensure and current registration to practice as a Registered Professional Nurse in New York State.
