Job Description
Kick start your career here! Actalent is seeking a multiple full time Social Workers for a client both on Oahu and Big Island.
Responsibilities
Advocate for and support patients by conducting all required patient assessment and care planning activities, including assessing new patient psychosocial needs and completing the KDQOL in accordance with company policy and state/CMS regulations.
Identify and counsel on psychosocial issues, providing patient and family education.
Coordinate communities of support for patients and their families, identify social agencies and other resources (e.g., financial/funding), provide information and referrals, and coordinate transient arrangements.
Represent patients as needed with appropriate local, state, and federal agencies.
Promote teamwork within the interdisciplinary clinic team, educate staff, and provide training around patient psychosocial care.
Collaborate with the Medical Director and physicians and maintain positive relationships with area hospitals, agencies, vendors, and the community.
Assist with clinical and operational processes to improve patient health and minimize missed treatments and hospitalizations, achieving target goals for patient outcomes.
Participate in monthly Quality Assessment and Performance Improvement (QAPI) activities and ensure compliance with federal, state, and local laws and regulations.
Essential Skills
Master's Degree in Social Work accredited by the Council of Social Work Education (CSWE).
Current licensure (in good standing)
Demonstrated working knowledge of the English language and ability to communicate verbally and in writing.
Basic computer skills, including Microsoft Office (Word, Excel, Outlook).
Pay and Benefits
The pay range for this position is $76000.00 - $95000.00/yr.
