Join our team at Preferred Family Healthcare.In this role, you'll be responsible for tracking client activity, submitting reports to the Department of Mental Health, and serving as a resource for the Healthcare Homes team and other clinical teams. Your contributions will ensure the smooth operation of the Healthcare Homes program and compliance with all relevant regulations. Key Responsibilities:

  • Provide general clerical support to the Healthcare Homes (HCH) Director and Team as needed.

  • Assist in distributing and coordinating reports to the HCH Team, Community Support Specialists (CSS), and community providers.

  • Serve as a Cyber Access Practice Administrator, responsible for maintaining user accounts, monitoring agency usage, and prescriber reports, and promoting the use of Direct Inform.

  • Provide technical assistance to the HCH Team and CSSs on the use of Care Manager and Cyber Access.

  • Track and ensure all required client screenings and reporting measures are completed and submitted appropriately.

  • Attend state-required meetings and trainings.

  • Maintain a monthly log of HCH activities and complete the monthly HCH Team Log as needed.

  • Prepare reports to be submitted to the HCH Director and Assistant Director before submission to the Department of Mental Health (DMH) each month.

  • Respond to Quality Assurance procedures, cooperate with utilization review processes, and follow the Clinical Records Department's established procedures, rules, and regulations.

  • Perform other special functions as assigned.

Education and/or Experience Qualifications:

  • High school diploma or GED required.

Knowledge, Skills, and Abilities:

  • Competency in Microsoft Office and Microsoft Access.

  • Strong communication skills.

  • Excellent organizational skills.

  • Ability to implement new and innovative programs.
