Job Summary:

The Primary Care Social Worker will be a key member of the care team, providing comprehensive psychosocial support to patients within the primary care setting. This role focuses on assisting patients in overcoming social barriers to healthcare, connecting them with community resources, and supporting their overall well-being. The ideal candidate will have strong clinical skills, excellent communication abilities, and a passion for working with diverse patient populations.

Key Responsibilities:

• Conduct comprehensive psychosocial assessments to identify patient needs and develop individualized care plans.

• Collaborate with primary care providers, nurses, and other healthcare professionals to ensure a holistic approach to patient care.

• Provide counseling and crisis intervention for patients experiencing emotional distress, mental health issues, or life transitions.

• Connect patients with community resources such as housing assistance, food programs, transportation services, and financial aid.

• Advocate for patients by coordinating care and addressing social determinants of health that impact medical outcomes.

• Support patients in navigating healthcare systems, insurance issues, and access to treatment.

• Facilitate support groups or educational workshops as needed.

• Maintain accurate and timely documentation of patient interactions and care plans in compliance with HIPAA and organizational policies.

Qualifications:

• Education: Master’s Degree in Social Work (MSW) required. Licensed Clinical Social Worker (LCSW) preferred.

• Experience: Prior experience in a healthcare or primary care setting is strongly preferred.

• Skills: Strong knowledge of community resources, excellent interpersonal and communication skills, ability to work independently and as part of a team, and proficiency in electronic medical records (EMR) systems.

• Certifications: LCSW or equivalent state licensure required/preferred.

The salary for this position is competitive and commensurate with experience. The pay range for this role in the state of Colorado typically falls between $36.05 - $40.86 hourly with the potential for performance-based bonuses and other benefits. Actual compensation may vary based on factors such as qualifications, experience, and location within the state.
