This position will be onsite at assigned skilled facilities. The Skilled Facility Navigator CAC Position is responsible for assisting our Optum NV Medicare Advantage members at the facility bedside upon admission to the skilled facility up through discharge focusing on ensuring a safe discharge plan and preventing a return to acute facility admission for our higher risk population. Will collaborate and work closely with Optum SNF’s and facility personnel.

This position is full-time. Employees are required to work an 8-hour shift schedule during our normal business hours of 8:00am – 5:00pm PST, Monday – Friday. It may be necessary, given the business need, to work occasional overtime or weekends. Our office is located at 2716 N Tenaya Way Las Vegas, NV.

We offer up to 90 days of on-the-job, field based training. The hours during training will be an 8-hour shift between 8:00am to 5:00pm, Monday – Friday. Training will take place at our office located at 2716 N Tenaya Way Las Vegas, NV as well as in the field.

Primary Responsibilities:

  • Manage intake of SNF members

  • Work with SNF facilities and the clinical team to manage requests for services from members and/or providers

  • Process incoming and outgoing referrals, and prior authorizations, including intake, notification and census roles

  • Document and assist the clinical staff with setting up documents/triage cases for Clinical Coverage Review

  • Handle resolution/inquiries from members and/or providers

  • Other duties as assigned

Required Qualifications:

  • High School Diploma / GED OR equivalent years of experience

  • Must be 18 years of age OR older

  • 1+ years of patient service/health care experience

  • 1+ years of experience within the health care Industry working in a hospital, physician's office, OR medical clinic setting and with health care insurance

  • Experience with computers and windows-based programs including Microsoft Word, Microsoft Excel and Microsoft Outlook

  • Ability to work full-time. Employees are required to work an 8-hour shift schedule during our normal business hours of 8:00am – 5:00pm PST, Monday – Friday. It may be necessary, given the business need, to work occasional overtime or weekends

Preferred Qualifications:

  • 2+ years of clinical customer service experience

  • Previous Clerical or administrative support experience

The hourly range for this role is $16.88 to $33.22 per hour based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
