The Certified Diabetes Care and Education Specialist (CDCES) receives direction operationally from the Endocrinology LVN Manager and is supervised by Endocrinology clinicians and the Associate Medical Director. Works collaboratively with Primary Care Physicians and Endocrinology Team focusing on meeting the educational needs of patients with type 1, type 2, and gestational diabetes. Supports overall vision of the Optum patient experience by achieving a positive balance of health care, cost outcomes and patient satisfaction of patients with diabetes. Promotes person-centered care, driving integration, a focus on behavioral health, leveraging technology, integrating related conditions and achieving the quadruple aim of population health, quality of the patients care experience, cost associated with care, and meaningfulness of the provider’s experiences. Ensures standards of care are implemented and in compliance with all state and federal regulations, standards and practice and Optum policies and procedures.

Primary Responsibilities:

  • Responsible for the coordination of care for the diabetic patient. Develops / reviews individual needs assessment based on standards set by the Association of Diabetes Care & Education Specialists (ADCES)

  • Reviews technology devices with diabetic pts (insulin pumps, continuous glucose monitors, etc.) and collaborates with ordering provider

  • Provides telephonic communication to support education process as needed

  • Utilizes approved clinical criteria to assess and determine appropriate level of care for the diabetic patient

  • Maintains ADCES

  • Supports Endocrinology Department initiatives focused on the care and research of patients with diabetes

Required Qualifications:

  • Registered Nurse (RN) license

  • Certified Diabetes Care and Education Specialists from the National Certification Board for Diabetes Educators (NCBDE)

  • Proven intermediate proficiency with Microsoft Office

Preferred Qualifications:

  • Bachelors’ degree or higher in nursing, public health or other related field

  • 1+ years of clinical experience working with the diabetic population

The hourly range for this role is $28.61 to $56.06 per hour based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
