YAI has an opening for a Social Worker in the Hankering For More Socialization Program. Some requirements necessary include being open minded. Creative and forward thinking. Love nurturing social connections and working directly with people. Have a high level of insight and emotional intelligence. Have experience in the ID/DD field.

Key responsibilities:

  • Develop social skills curriculum with innovative and evidence-based practice

  • Co-facilitate integrated community outings on, Facilitate weekly social skills groups that support HFM members' work to actualize social plans with friends

  • Provide supervision for student interns studying social work or mental health counseling

  • Contribute clinical insight & support during team meetings and support individual members with their personal goals

  • Maintain quality assurance documents including detailed case notes for each clinical interaction and records on goal progress in compliance with Medicaid Waiver regulations

  • Support group members in developing community connections and social capital while in integrated settings

  • Communicate & collaborate with members' circles of support to ensure optimum goal achievement and coordination of care (i.e. attend bi annual ISP/Life Plan meetings)

  • Maintain YAl annual staffing requirements in compliance with OPWDD regulations and standards and other agency wide initiatives as needed

  • Provide clinical insight and training to the larger team in collaboration with program leadership. Facilitate clinically focused specialty groups during weekday evening(s) for program members including sexuality, advocacy & rights, social skills etc

  • Work with Manhattan Self Advocacy leadership in developing an annual schedule of monthly general assembly meetings in addition to monthly board meetings (2 meetings per month)

  • Coordinate and establish quarterly YAl Self Advocacy Summits with the approximately 10 regional groups along with quarterly corresponding self-advocacy staff support meetings

  • Support self-advocacy presence and engagement at inter agency committees and initiatives

  • Respond to agency requests for self-advocates to participate in special events (Leadership Summit, United Nations, etc.)

Minimum Qualification Requirements including education, experience, licensure/certification, etc. and essential physical capabilities (e.g., lifting, assisting lifting, standing, etc.)

  • Master's degree in field of psychology, social work or related field (as specified in OPWDD regulations)

  • Have at least 1 year of experience facilitating groups within the ID/DD field.

Additional Knowledge, Skills and Abilities:

  • Hold a SIFI credential (or are eligible to pursue) to provide field instruction for social work student interns

  • Demonstrate excellent problem-solving skills and an ability to overcome challenges

  • Have strong leadership qualities

  • Are intrinsically motivated and a self- starter

  • Possess strong organi7.ation and communication skills

  • Demonstrate exceptional clinical skills (group work and individual) and an ability to support adults in moving beyond barriers

  • Are open to provide and receive feedback from colleagues, team members, and group members

  • Are committed to the compliance and quality assurance activities affiliated with the YAI

  • Network and OPWDD funding sources
