BCforward is currently seeking a highly motivated Care Management Support Coordinator II for an opportunity in Remote!

Pay Range: [\$17] - [\$19]

Job Description:

Position Purpose:

The Administrative Care Coordinator supports administrative care management activities by performing outreach, answering inbound calls, and scheduling services. This role serves as a key point of contact for members, providers, and internal teams to resolve issues while ensuring compliance with state and regulatory guidelines.

Key Responsibilities:

  • Conduct outbound calls to members to assist with care plan follow-ups, provide educational resources, and address scheduling concerns.
  • Answer inbound inquiries from members and providers, explaining care plan procedures and available resources.
  • Facilitate connections between members and health plan or community resources to enhance service quality.
  • Support onboarding activities, including sending welcome letters, program materials, and related correspondence.
  • Maintain accurate, non-clinical member records in compliance with policies and regulatory requirements.
  • Identify and refer members to available benefits and local resources to address Social Determinants of Health (SDOH) needs.
  • Work collaboratively within a cross-functional call center team to provide timely and efficient service.
  • Perform other administrative and support duties as assigned.

Education & Experience:

  • High School Diploma or GED required.
  • 1-2 years of experience in a related role.
  • Call center experience with a strong focus on customer service preferred.
  • Experience in the healthcare industry (e.g., medical office, medical assistant, customer service in a healthcare setting) is a plus.

Skills & Qualifications:

  • Strong communication and customer service skills.
  • Ability to navigate multiple computer systems efficiently.
  • Detail-oriented with strong organizational skills.
  • Experience with call center metrics such as Average Handle Time (AHT), Service Level Agreement (SLA), and Quality Assurance.
