POSITION SUMMARY: This position will provide Community Health Worker (CHW) services for people that are part of our Community Support Program (CSP) and Encampment projects. These projects primarily serve individuals who are or recently have experienced homelessness or housing instability, are justice-involved and/or are working to recover from diagnosis that are impacting their lives. The CHW will provide diagnosis-related education interventions that focus on homelessness prevention, early intervention, and service coordination. The CHW will meet with participants individually and facilitates groups in community-based settings and at partnership locations.
