The position for a Nurse Practitioner, as defined in the Minnesota Nurse Practice Act; this provider functions as a direct care provider. The clinical mission of CUHCC is to provide healthcare to underserved populations and to alleviate disparities in health care outcomes. Additionally, CUHCC is a clinical rotation/practicum site for the University Of Minnesota School Of Nursing. This is a 75% time appointment. Reporting Relationships Direct supervision by Medical Director. The position has no staff supervisory responsibilities. The position may precept under-grad and/or graduate nursing students and medical students in clinical rotations at CUHCC. Essential Functions and Percentage for Each Function Primary Care 85% Provide primary medical care to adults, children and teens, within the scope of the MN Nurse Practice Act and MN. Board of Nursing regulations and with the collaboration of a physician as outlined in state law. The scope of practice for the Advanced Practice Clinician- NP may include, but is not limited to the following for which privileges are granted annually, and in accordance with the written agreement with the collaborating physician: Taking patient histories. Performing physical examinations including well women annual examinations. Interpreting and evaluating patient data. Ordering or performing diagnostic and/or therapeutic procedures which may include, but are not limited to biopsy, minor procedures and wound care. Prescribing, administering, and dispensing prescription and nonprescription medications and medical devices. Diagnosing and treating illnesses, infectious diseases, minor trauma, and chronic conditions or diseases. Making patient referrals to other providers within and outside of CUHCC to facilitate treatment plans. Providing contraceptive and sexually transmitted infection care services. Educating patients on health promotion, disease prevention, current treatments, importance of patient compliance, and answering patient questions. Arranging follow-up to review lab or test results with patients, via appointments, phone calls, letters, or mychart messages. Working with other providers on the care team, when indicated, to coordinate care of more complex patient problems Consult with other physicians and/or health care providers as needed to confirm difficult diagnosis and facilitate appropriate treatment plans. Ensure patient's beliefs and values are respected and empower patients to actively participate in their care Other responsibilities as needed Quality Program and Other Initiatives 10% Participate in quality and care integration initiatives. Demonstrate evidence of continuing professional growth by involvement in peer review and the review of protocols and procedures; participates in continuing education. Precept students from diverse backgrounds and professions to help develop a strong integrated educational model. Practice Collegiality: Be helpful, respectful, approachable, and team oriented; build strong working relationships and a positive work environment To attend clinic/staff functions (staff meetings, retreats, in-services Abide by the Health Services Expectations for Professional Behavior Maintain patient confidentiality. Maintain a polite and respectful attitude toward all students and staff Meet all on-going requirements for professional licensure and certification To perform other work-related duties as assigned Emerging projects 5% Qualifications Required Qualifications Master's degree plus four year of nursing experience or a doctorate plus at least one year of nursing experience. Current unrestricted license to practice as an Advanced Practice Registered Nurse in Minnesota. Up to date board certification Preferred Qualifications Bilingual in Spanish, Somali, Hmong, Vietnamese, Lao. Experience in providing nursing care to diverse populations. Experience in community health center-type settings. About the Department CUHCC serves nearly 11,000 patients a year through over 55,000 visits annually.
