POSITION SUMMARY: Works with people with qualifying Physical/Mental Health/Substance Abuse/Developmental Disability diagnoses and their natural supports to link/monitor/refer to services that meet their individual needs and personal goals. Also provides ongoing advocacy.

QUALIFICATIONS: Minimum Education, Training & Experience: Associates' Degree in a Human Services related field. One to Two (1-2) years of relevant experience in Human services (including but not limited to Mental Health, Social Work, Substance Use, Education, etc.)., with a minimum of 6-12 months experience developing Care Plans/SMART Goals and functional knowledge of community resources. **For applicants with an Associates degree, a waiver is required to be completed and accepted by The New York State Department of Health before hire.
