Position Summary

Lincoln County Behavioral Health is looking for qualified individuals who are looking to make a difference in the lives of our community members.

The person in this role will participate in a multi-disciplinary team providing specific services to individuals and/or family clients, as well as community partners also involved in the client's service plan.

This position is an important part of providing linkages and referrals to community resources for people who need them but may not be aware of what we offer.

Essential Functions/Major Responsibilities

Assist in the development and implementation of the child and family team, this includes:

  • Maintaining regular contact with the child, family, service providers, and representatives of other systems in which the child is involved.

  • Provide case management services such as assessing needs, linking to services, monitoring, and coordinating services from multiple providers and facilitating crisis response.

  • Maintaining the child’s Integrated Service Array case file and is responsible for gathering and transmitting fidelity measures and utilization, family satisfaction and outcome data.

  • Collaborate with the child and family team in developing and implementing a transition plan from the Integrated Service Array.

Provide linkages and referrals to families, including:

  • Create linkages to and monitors transitions between levels of care.

  • Provide support and consultation to families about system involvement and advocacy.

    Other functions:

  • Interact and convene with community partners and resources as necessary to assure population needs are being met.

  • Lead, convene, and participate in team meetings, trainings, and activities to further the health of the community.

Typical Qualifications

Minimum Education Level

Bachelor's Degree in a social behavioral field with special training in behavioral health.

Minimum Experience Level

Two- Three years of clinical treatment experience in the behavioral health field.

Certifications

Specifics: Classified as a Qualified Mental Health Associate.

Driver’s License

Oregon Driver's License required within 30 days of hire.

Desirable Qualifications: Bilingual and/or bicultural competencies for Latino population.

Lincoln County offers a comprehensive benefit package including vacation, holiday, and sick leave as well as medical, dental, vision which is paid at 90% by the County and 10% by the employee. Other fully paid benefits include life, AD&D, long-term disability insurance. Other optional insurances are available. A 401K retirement plan is also provided. The County's current contribution rate is 11%.

Regular and Trial Service employees working between 20 - 39 hours per week on a regular basis are eligible for pro-rated employee benefits.
