JOB SUMMARY Performs the social work functions of bio-psycho-social assessment, short term crisis intervention, supportive counseling and resource referral. Provides intervention to assist patients/families to improve social and economic difficulties that may interfere with health and wellness. Facilitates discharge planning across the continuum of care. Education:Bachelors degree in Social Work or related human services field. MSW preferred. Experience:Previous social work or human services experience. Two (2) to Three (3) years of health care experience preferred. KNOWLEDGE, SKILLS AND ABILITIES * Ability to read and understand oral and written instructions, and effectively communicate information. * Basic computer skills (including opening/reading email, basic internet use). * Excellent interpersonal skills necessary to communicate with departments, employees, physicians and managers. * Ability to relate to people with diverse educational and cultural backgrounds. * Must be organized, flexible and a team player. * Ability to manage time and meet deadlines in a hectic work environment. * Working knowledge of windows based software (including word processing, database and spreadsheet) for occasional performance of duties. * Requires the ability to take the appropriate action in response to changes, circumstances or problems. * Requires high level of analytical ability in order to gather and interpret information from various sources. * Ability to formulate and implement a viable plan for intervention.
