Title: Care Manager (RN) Duration: 3 Months (Possibility to extend or convert) Remote (Telephonic, minimal in-person visits for NE region.) Position may require an in-person visit for assessing members for a HENS/Waiver. Will be working with members new on BHP plan Daily schedule & OT expectations - M-F 8am-5pm. No OT expectations. Description: Develops, assesses, and facilitates complex care management activities for primarily physical needs members to provide high quality, cost-effective healthcare outcomes including personalized care plans and education for members and their families. Education/Certification Associates Degree of Nursing. Preferred: Associates Degree of Nursing.
