Community and Enrollment Liaison

Shift: Mon to Fri, 8 am to 430 pm, no nights/weekends/holidays

Status: Full time

Location: Coatesville, PA

Position Purpose:

Responsible for developing relationships with community referral sources, agencies, healthcare/human services professionals, and families to provide greater understanding of the PACE Organization (PO). Responsible for obtaining referrals for people qualified and interested in the PO program.

Serves as the contact for newly qualified referrals, providing overview of PACE model and obtaining initial information relating to clinical and financial eligibility during the home intake visit. Must accurately document all activity to manage qualified referrals through the entire enrollment process in both the Customer Relationship Management Platform (CRM) and Electronic Health Record.

Position Details:

Mercy LIFE Chester County is a new Trinity Health PACE program opening in Coatesville, PA.

What you will do:

  • Initiate, build relationships with and maintain contact with community referral sources to promote the PACE program.

  • Work collaboratively to enhance program brand in the community, identify sales opportunities, and ensure operations/sales growth strategies are aligned.

  • Act as a liaison between participants and Enrollment Team.

  • Enters documentation into Customer Relationship Management Platform (CRM) and Electronic Health Record.

  • Serves as contact for prospective enrollees and meets with them in homes complete intake assessment to determine eligibility and interest.

  • Responsible for reviewing enrollment documentation and ensuring potential participants understand program services.

  • Coordinate and continually monitors each component of enrollment process.

  • Assist orr submits Medicaid applications for required PACE applicants. Reviews Medicaid eligibility and provides administrative facilitate smooth application and certification.

  • Communicate regularly with PO team members sharing information the enrollment process as needed.

  • Advocate within the organization for the customer service needs of the referral community.

  • Provide in-services and presentations to the community on program

Minimum Qualifications:

  • Bachelor’s degree with a minimum of two (2) years of sales or marketing experience is preferred. A combination of education and experience may be acceptable.

  • Valid driver's license, insurance and use of personal vehicle.

  • Superior written/verbal communication, customer service and organizational skills.

  • Proficiency with MS Office.

  • Must manage conflict well, have high decision quality and flexibility to adapt to ongoing change.

  • Strong critical thinking skills as well as independent decision-making abilities and ability to prioritize workload.
