Penn Medicine is seeking a full-time, day shift Graduate Pharmacist for Specialty Central Fill at its 76th Street location by the Philadelphia airport.
Summary:
Graduate pharmacists are required to provide pharmacy services to multiple areas throughout the hospital. They must possess skills to cover all aspects of pharmacy operations and make sound clinical and operational decision regarding medication management while in the presence or under direct supervision from a PA licensed pharmacist. The pharmacist must actively be pursuing pharmacist licensure in the state of Pennsylvania. Tasks that may be performed in the presence of a PA licensed pharmacist include: designing, recommending, monitoring, evaluation and documenting patient and age-specific pharmacotherapy; providing concise, applicable and timely responses to requests for drug information to ensure optimal drug use and medication-use education; managing medication safety issues; monitoring medical and nursing compliance; provide medication education to patients and caregivers; participate in discharge rounds and review discharge medication orders; and ensuring adherence to the hospital's medication use policies, procedures and relevant regulatory agencies. Under the direct supervision of a PA licensed pharmacists, the graduate pharmacist may additionally verify medication orders, check and dispense pharmaceutical products and respond to code call and rapid response emergencies.
Responsibilities:
Provide optimal pharmacy services to patients and other health care professionals by designing, recommending, monitoring, evaluating and documenting patient and age-specific pharmacotherapy
Demonstrate an awareness of patient and employee confidentiality and safety when carrying out daily responsibilities
Provide concise, applicable and timely responses to drug information requests from health care providers and patients
Under direct pharmacist supervision, verify, prepare and dispense medications using appropriate techniques to ensure positive outcomes of drug therapies and compliance with national and health system’s policies and procedures
Participate in the management of medical emergencies under direct pharmacist supervision
Review patient discharge medications and provide discharge medication education to patients and caregivers
Ensure adherence to medication-use policies and guidelines
Manage time effectively to fulfill practice responsibilities
Education or Equivalent Experience:
Bachelor's of Science or PharmD from an accredited College of Pharmacy(required)
Ability to obtain PA pharmacist license within 90 days of hire (required)
