As a Case Manager you will be responsible for working as a member of the care team using principles of assessment, care planning, service and community referrals, and evaluation to coordinate family-guided, youth driven care for referred children/youth/families. Level commensurate with education and relevant work experience.

Responsibilities:

· Screen, assess and as required, perform assigned daily on-call phone triage and walk-in support to assigned youth/families

· Provide supportive counseling and case management services to referred youth and families including coordination of services with community agencies

· Effectively identify and use available resources leveraging knowledge and awareness of agency and community services, and assume an influential role in building recommended service options

· Advocate and broker for clients in making referrals to agency services and community programs

· Use knowledge, personal influence and persuasion to encourage and facilitate connection of youth and their families with continuing care services

· Establish and maintain productive work relationships with community service providers to facilitate effective referrals and service evaluation

· Maintain required contact with youth and their families and conducts family/foster family meetings

· Serve as a liaison between the program and other internal and external resources

Requirements:

• Minimum of a High School Diploma or GED. Bachelor’s degree in Human Services or a related field of study preferred.

• 2+ years of human service experience.

• Must have a valid NYS Driver’s License and meet agency driving and insurance requirements.

  $17.50 Minimum Rate per hour, $25.49 Maximum Rate per hour, based on experience.
