SUMMARY:

The Outreach and Enrollment Worker offers individuals and families information and assistance with enrollment into Qualified Health Plans (QHPs), Medicaid, Medicare and Childrens Health Insurance Program coverage options. Develops, implements and coordinates in-reach and outreach activities designed to help eligible health center patients and service area residents enroll into coverage options. Represents Family Health Services at community outreach events, and identifies new collaborative efforts and partnerships to advance outreach goals. Completes required federal and/or state consumer assistance training to provide fair, accurate, and impartial assistance.

MINIMUM QUALIFICATIONS:

  • High school diploma or GED equivalent
  • Experience within community service, health or social service sector
  • Excellent communication skills, both oral and written
  • Fluency in English; bilingual English and Spanish preferred
  • Proficient with Microsoft Office Programs
  • Knowledge of local community resources preferred
  • Must be able to exercise discretion in all matters related to the enrollment process
  • Ability to work independently and work collaboratively as member of a team
  • Ability to work flexible schedule
  • Valid Idaho Drivers License
  • Ability to pass a background check
  • Properly insured, reliable vehicle

DUTIES AND RESPONSIBILITIES:

  • Provide comprehensive and culturally-sensitive information about Idaho Health Insurance Marketplace and public insurance programs and benefits
  • Assist applicants in properly completing and submitting applications
  • Conduct follow-up with individuals and families on the progress of their enrollment and help troubleshoot enrollment barriers
  • Track provided eligibility assistance and successful enrollments in QHP, Medicaid, Medicare and CHIP insurance plans
  • Ensure the confidentiality of all applications, records, and information
  • Serve as a point of contact for individuals and families contacted during off-site outreach activities regarding health insurance enrollment
  • Support outreach and enrollment efforts by working collaboratively with community partnersschool nurses, public health programs, churches, labor groups and other community entities.
  • Participate in outreach community events as needed
  • Assist in planning of new strategies for outreach
  • Comply with and successfully complete federal and state consumer assistance training required for personnel carrying out consumer assistance functions
  • Participate in ongoing trainings, learning sessions, conference calls, webinars and other professional development opportunities
  • Document and maintain records of encounters with individuals and/or families
  • Ability to travel to outreach and enrollment sites

It is our policy to properly determine the accuracy of reporting and billing our services in accordance with he rules, laws and regulations of FHS, the state government, and the federal government. FHS employees will do their part to ensure accurate documenting and billing practices, participate in compliance trainings and will identify and report any concerns or activities that may violate these standards.

Each employee will be trained on the FHS Medicare Compliance Plan and the Standards of Conduct and asked to sign a Conflict of Interest Statement at hire and annually thereafter. A copy of the Medicare Compliance Plan and the FHS Standards of Conduct are available on the FHS home page under the Compliance tab.

Family Health Services is committed to providing our patients with the highest standards of care by becoming recognized as patient centered medical home.
