Schedule: Monday - Friday - 8 hour shift - Hours can be between 12:00 PM to 12:00 AM

Job Summary and Qualifications

The ER Discharge Coordinator/Point of Care Scheduler is a non-clinical role is responsible for Point of Care (POC) Bedside scheduling as part of the company’s centralized ER and Urgent Care Follow-up programs. The truest intent of PoC scheduling is a bedside initiated patient experience, with the objectives of increased contact rate, scheduling immediacy, and improved overall patient experience. The PoC Scheduler creates and maintains a positive customer experience in a patient centered, quality driven environment.

In this role you will:

  • Speaks with customers, patients and stakeholders in a pleasant and professional manner; Communicates appropriately and clearly with department leadership, co-workers and customers/patients.

  • Schedules all patients for follow-up care bedside when appropriate.

  • Ensures performance and quality standards are met.

  • Is receptive to constructive feedback and coaching; Demonstrates the ability to be kind, compassionate, considerate, approachable, friendly and open minded.

  • Provides superior customer service to internal and external customers and patients; Supports the vision and strategy of the organization, positively promotes the services offered, and elevates issues to leadership as appropriate.

  • Suggests ideas for positive changes to department policies and procedures.

  • Practices and adheres to “Code of Conduct”.

  • Maintains confidentiality and HIPAA compliance on all internal and external channels.

  • Easily adapts to changes in work environment or job assignment; Exhibits willingness to master new work routines and methods.

  • Complies with documented attendance/punctuality policies.

Qualifications:

  • High school diploma or GED preferred.

  • One year of related experience preferred.

  • Previous Scheduling experience preferred

  • Experience using computers and tablets

  • Must be willing to work bedside.

  • Bilingual is a plus
