RN: Registered Nurse Case Manager

Full Time Monday - Friday with on call responsibilities M-F and rotating Weekends

Experience Based Pay Rate between $38.50-39.42/hour + weekend on call rate

What will I do as a RN Case Manager f or The Wesley Community?

  • Support the Program Director and promote good-will by interpreting Service and Facility Policies and Procedures to residents, families, visitors, and personnel.

  • Assist in directing the day-day functions on the unit, in accordance with current rules, regulations and guidelines that govern the facility. Maintaining strict adherence to the Department of Health Regulations. Will ask Program Director for assistance or clarification as needed.

  • Participate in surveys/inspections made by authorized government agencies.

  • Make written and oral reports/recommendations to the Program Director, as necessary/required, concerning the operation of the unit.

  • Share the responsibility of assuring that supplies and equipment are properly used, stocked and available on the unit; equipment is in good working order and location of supplies and equipment is familiar to staff.

  • Monitor and maintain the appropriate controls on the resident medication system and verification of the MAR's. Check for any label discrepancies, expired medications and accuracy of orders written versus label directions.

  • Provide leadership which stimulates and encourages a team approach to promote interest and pride in high quality resident care.

  • Help develop work assignments for personnel according to skills and training in relation to resident needs based on the individual service plan.

  • Assist in assuring continuity of good quality care of residents on all shifts by establishing and initiating 24-hour comprehensive Individualized Service Plans.

  • Modify service plans to meet changing resident conditions every 6 months and as necessary.

    RN Case Manager Requirements :

  • Graduation from an accredited School of Nursing; Associate or bachelor's degree.

  • Must have a current NYS RN license.

  • 4 years of RN experience with geriatric population, preferred.

  • 2 years case management experience, preferred.
