Special Work Conditions: This person will serve as a Health Service Worker. The position will be housed inWhiteCounty, but the individual will cover several counties in the area.
Performs Health care case management duties – reviews health care policy and forms; attends staffings as needed; gathers and reviews medical records; assists in the completion of adoption packets; ensures all children assigned have active Medicaid; orders birth certificates and social security cards for each child that enters care. Health service workers confer with caseworkers and caretakers; transport children to appointments; schedule medical appointments; comply with PACE evals by developing comprehensive health assessment summaries and coordinating UAMS appointments; complete a portion of the medical passport pertaining to health services; document medical services in CHRIS; scan medical reports into eDoctus; send medical information to appropriate parties; conference with HS supervisor regarding care issues and concerns; assists and support care givers.
Preferred Qualifications : Knowledge of basic computer functions and operations. Ability to learn new computer skills. Ability to prepare and present oral and written information. Ability to stand or sit for extended periods of time.

Summary
The Department of Human Services (DHS) Program Assistant is responsible for providing social services to program recipients and support for families. This position is governed by state and federal laws and agency/institution policy.
Functions
Teaches and demonstrates living skills to clients, such as budgeting, cooking, menu planning, housekeeping, and child care. Transports clients and/or family members for medical, dental, educational, and counseling appointments. Aides clients in locating resources by contacting organizations or agencies for assistance. Assists with developing and monitoring behavior plans, parenting/anger management classes, and maintaining narratives detailing action taken on behalf of the client/family. Participates in training clients in employment skills by providing materials and tools, monitoring work assignments, and providing direct assistance when needed. Attends staff and mandatory trainings. Schedules, monitors, and documents family visits. Performs clerical duties, such as answering the telephone, routing calls, receiving referrals for clients, typing documents, filing, and handling mail. Performs other duties as assigned.

Minimum Qualifications
The formal education equivalent of a high school diploma; plus one year of experience in a social-service related field. Additional requirements determined by the agency for recruiting purposes require review and approval by the Office of Personnel Management.
