WellLife Network Inc. is hosting an Interview Day onWednesday, April 9, 2025, from 10AM – 2PM. We are seeking passionate and reliable Care Coordinators for our Long Island, Suffolk County Care Coordination Program. The Care Coordination Programs services clients with mental disabilities to promotes health and wellness. The role ofthe Care Coordinatoris to provide a person-centered, recovery-focused system of care for individuals.

Full-time Union position budgeted at $25.10 per hour with Excellent benefit and generous PTO!

job Qualifications

• Bachelor’s Degree in a human Service field

• 2 yrs. Human Services experience

• Valid NYS Driver’s License and access to motor vehicle

• Strong computer skills and knowledge of Microsoft Office (e.g. Word, Outlook, Excel, etc.).

• Strong interpersonal skills

• Strong written and verbal communication skills.
