CenterWell Senior Consultative Pharmacist/Centralized Refill Team Orlando in Tallahassee, Florida

The Senior Consultative Pharmacist consults with patients, pharmacies, and providers to provide patient care in the form of optimal medication therapy. The Senior Consultative Pharmacist work assignments involve moderately complex to complex issues where the analysis of situations or data requires an in-depth evaluation of variable factors.

The Senior Consultative Pharmacist advises safe, accurate and timely usage of medications. The Pharmacist role will collaborate with Primary Care Providers in managing the prescription refill process with the goal of delivering enhanced medication management services. The pharmacist will serve as part of the clinical team and is responsible for managing patients' drug therapy to ensure optimal, cost-effective care. This role supports the full physician panel regardless of health plan and may cover multiple clinic locations. Monitors medication therapy and adjusts as needed. Works as the medication expert in focused proactive heath campaigns that target improving health outcomes for patients with specific conditions.

Senio Consultative Pharmacist Role Specific Responsibilities

  • Comprehensive oversight of maintenance medication renewals for senior patients, including collection and interpretation of patient-specific data, drug-related laboratory results, and identification of patient-specific goals of therapy

  • Communicate and collaborate with Primary Care Providers, clinic staff, and dispensing pharmacies regarding patient care needs.

  • Execute clinical protocols as allowed per organization and state regulatory agencies.

  • Optimize drug coverage benefits for appropriate care and reduced costs (preferred formulary products, lower cost alternatives and home delivery pharmacy)

  • Consult with patients and providers in the management of chronic conditions.

  • Provide patient education on appropriate use of medications and self-management skills as needed.

  • Supervise pharmacy technicians; intervene when necessary to ensure optimal patient care.

  • Participate in medication-related cost & quality initiatives based on organizational needs.

  • Document all activity within EMR and other required pharmacy or clinical management systems.

Additional Information

Required Qualifications

  • Active and unrestricted pharmacist license within the state(s) of the provider practice locations

  • Pharm.D. or other advanced pharmacy degree.

  • 2+ years' experience as a practicing pharmacist or equivalent residency experience

  • Experience in managing drug therapy for patients with chronic conditions.

  • Excellent organization and communication (verbal and written) skills.

  • Ability to work in a team environment and support common goals.

  • Strong attention to detail; able to concentrate without distraction in a fast-paced environment.

  • Ability to prioritize multiple tasks, anticipate and solve problems.

  • Intermediate level or higher competency with MS Office programs such as Word, Excel and Power Point

  • Ability to participate in Federal prescription programs.

  • Must be passionate about contributing to an organization focused on continuously improving consumer experiences and working with geriatric population.

  • Pharmacist must have a valid FL Consultant Pharmacist license or ​must complete a Collaborative Practice Agreement [ CPA] certification course before approval for CPA participation. The Continuing Education programs are offered by the University of Florida & Nova Southeastern University.

  • Be comfortable working in a REMOTE - HYBRID - HOME position.

Preferred Qualifications

  • Clinical residency in ambulatory care or similar discipline

  • Board certification in ambulatory care, geriatrics, or pharmacotherapy

  • Familiarity with electronic medical record (EMR) systems

  • Multilingual (highly preferred in both English and Spanish)

Senior Consultative Pharmacist Working hours:

Monday to Friday 8AM-5PM

Scheduled 40 hours per week

Local travel will be required; Mileage will be reimbursed

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.$102,200 - $140,700 per yearThis job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
