Must have the following to apply:

  • Bachelors degree from an accredited college or university in Social Work, Behavioral Science, or a related field
  • Three years experience providing direct case management social services including accessing clients needs and developing individual, comprehensive, long-term action plans for recovery utilizing a variety of resources OR
  • Any equivalent combination of training and experience that provides the required knowledge, skills, or abilities
  • Ability to meet attendance requirements
  • Due to the nature of the job, must speak, read, and write in English
  • Ability to communicate clearly and effectively both orally and in writing
  • Thinking skillsin order to maintain control of client caseload
  • Ability to work on multiple cases effectively and efficiently simultaneously
  • Limited amount of effort required associated with walking, standing, lifting, and carrying light objects (less than 25lbs.) 5-10% of work time

Job Description:

This position is responsible for interviewing, accepting, and providing comprehensive, long-term, structured, difficult case management services for an assigned caseload of clients participating in the Up and Out Program.

  • Understands clients history in order to determine most effective program plans
  • Develops comprehensive program plan/goals and evaluates clients progress by conducting mentoring and counseling sessions with client and/or staff
  • Serves as advocate for client in order to acquire services that will enable them to functionally cope with their environment
  • Ensures constant compliance with funding requirements

Work Conditions:

  • Duties are usually performed seated
  • Sitting may be relieved by brief or occasional periods of standing or walking
  • Work is performed in a normal office environment where there are minimal to no discomforts associated with changes in weather or discomforts with noise, dust, dirt, and the like
  • Work requires performance of duties in social services environments where exposure to communicable and infectious diseases is possible

Days and Hours:

Full-Time position. 37 hours per week. Complete work schedule will be discussed with applicant.

Pay:

$21.68 to $24.38 per hour, depending on experience. Benefits are offered.
