We are looking for an incredible Chaplain to join a magnificent Hospice team to provide vital spiritual counseling and guidance to our patients, families, and caregivers. We need someone who believes in our mission and philosophy and can provide compassionate services to a diverse population encompassing all faiths that are represented.

Qualifications:

• May have a degree from an accredited theological school, or an individual who by ordination or by ecclesiastical endorsement from the individual's denomination has been approved to function in a pastoral capacity.
• One year of chaplain experience preferred
• Experience working with death and the dying, as it relates to patients and their families/caregivers;
• Ability to work as a contributing member of interdisciplinary group
• Comfort level in inter-faith setting
• Assesses spiritual needs of patients and families/caregivers in the hospice program and ensures that appropriate services are rendered in a timely manner;
• Serves as liaison and support to community chaplains and spiritual counselors;
• Maintains accurate records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision; •
• Attends patient care conferences and interdisciplinary group meetings, providing consultation, education, and support
• Assists with the supervision of spiritual care volunteers when assigned to patients/families/caregivers;
• Actively participates in quality assessment performance improvement teams and activities.
• Must have and maintain a car to be used for work.
• Must have and maintain car insurance as required by state law.
• Has ready access to a telephone for receiving assignments or changes in assignments.

Job Type: Part-time

Pay: $27.00 - $35.00 per hour

Work Location: In patient homes and facilities in Salt Lake and Utah counties
