Location: Thornbury , Ontario 

Position: BSO  Personal Support Worker (PSW) - Permanent  Full Time

 Assigned one shift. 2 week rotational schedule 

Southbridge Care Homes is looking to add a caring, passionate, and enthusiastic Personal Support Worker to our amazing home, family, who can serve our lovely residents, to keep them safe, healthy, and happy. On the daily basis, our PSW will enhance direct personal and nursing care to residents to fulfill the physical, intellectual, emotional, social, and spiritual needs of our residents.

Duties and Responsibilities

  • Assist in providing direct care for residents.

  • Provide emotional and social support to residents/families, including palliative care.

  • Work with multi-disciplinary team members to meet the needs of residents.

  • Contribute in delivering person-centered care by promoting the abilities and self esteem of the residents, develop strategies to promote optimal quality of life for the residents with the responsive behaviors.

  • Conduct assessments using various assessment tools which describes behaviors of residents including observational behaviors, identifying behavioral risks, and identify caregiver strategies.

  • Demonstrate participation in quality improvement activities and provide monthly reports.

  • Assist in tasks by considering policies of the home and the behavioral support Ontario project.

  • Maintain amazing relationship with all the stakeholders including staff, residents and the general public.

  • Provide an excellent customer service to all the stakeholders and assist in the administrative duties of the home.

  • Other duties as required.

    Qualifications

  • PSW Program Certificate or diploma that meets the appropriate standards established by the Ministry of Training, Colleges and Universities, the National Association of Career Colleges, or the Ontario Community Support Association.

  • Has passion and heart for providing quality care.

  • Knowledge and experience in care planning systems and documentation required.

  • Excellent ability to communicate positively with other staff, residents, and families.
