RWJBarnabas Health is seeking a coordinator to work on our Transitional Care Unit at Community Medical Center.

Community Medical Center, is a 592-bed, fully accredited acute care hospital offering area residents world-class medical treatment with the comforts of hometown care. Opened in 1961, CMC has evolved into the state s newest teaching hospital with four residency programs and Ocean County s largest and most active health care facility caring for over 21,500 inpatients, 178,000 outpatients and 73,700 emergency department patients each year.

Qualifications:

  • RN License

  • Prior staffing/medical office experience, as well as supervisory responsibility, is preferred.

  • Computer applications including Word and Excel, strong communication / interpersonal skills and the ability to multi-task successfully.

  • Excellent verbal communication, interpersonal and demonstrated writing skills.

  • Proven record of accomplishment of compassionate care & superior service, and a commitment to providing healthcare excellence.

Essential Functions:

  • Manages the scheduling of assessments to appropriately capture the greatest burden of care which includes scheduling OMRA, PPS, Managed Care/Insurance and change of condition assessments according to applicable rules and regulations

  • Participates in the transmission to the IQIES database and validation of MDS acceptance

  • Reports pertinent observations and reactions regarding residents promptly to Supervisor and Physician as necessary

  • Assures compliance with policies and procedures

  • Reports problems and complaints timely to the Nurse Manager

  • Maintains positive working relationships between nursing and other departments

  • Supports and participates in the center's Performance Improvement initiatives as needed

  • Participates in Survey readiness

  • Completes records and reports as needed

  • Adheres to professional codes of ethics and maintains PHI confidentiality in the center and in external locations, including electronic social networks

  • Accurately completes the MDS assessments and Care Assessment Areas as allowed within the nursing practice act,

  • Performs patient interviews and assessments as well as gathers assessment information through documentation review and staff/family interviews to conduct quality assessments

  • Initiates, recommends, communicates and implements changes in resident's care plans

  • Assists with or institutes emergency measures for sudden adverse developments in residents

  • Documents care performed and observations of resident status in the clinical record promptly by utilizing standard clinical documentation guidelines as required by the facility, and local, state and federal rules and regulations

  • Monitors completeness and accuracy of own clinical medical record entries

  • Respects resident rights and follows facility s Abuse and Neglect Prevention Policies and Procedures

  • Immediately reports allegations to Administrator,

  • Notifies and assists the Nurse Manager with interventions and investigations as directed

  • Assures residents preferences and advanced directives are respected

  • Performs other tasks as assigned

  • Educates appropriate staff on state specific MDS related documentation requirements

  • Communicates educational needs of staff to staff development coordinator and supervisors

  • Participate in the orientation of new employees
