Work Type: Full Time

Hours Per Week: 40.00

Work Schedule: 8:00 AM to 4:30 PM

Summary:

This position is accountable to provide the following functions: Nutritional care to hospital patients and nursing home residents through the application of current knowledge in the field of practice, Back-up dietitian for any of the following areas: Inpatients, Diabetes Self-Management Education (DSME), MNT Clinic outpatients and/or associated Family office practices as assigned.

Responsibilities:

  • Position Competency and Knowledge of Essential Job Functions.

  • Responsible for timely, professional nutrition assessment, evaluation, intervention, recommendations, follow-up, education/ counseling and documentation (MNT) for inpatients, outpatients, residents, and significant others as appropriate. Recommends modifications to Nutrition Care Plan to meet patient specific needs.

  • Provide individual and group patient nutritional counseling, teaching, and follow-up in Diabetes Self-Management Education (DSME) and outpatients as assigned

  • Collaborate with health care professionals on the assessment of complex patients.

  • Monitor nutritional screening. Evaluate patient tolerance to therapeutic diets, when appropriate.

  • Participate in multi-disciplinary meetings, consult with staff, and communicate effectively.

  • Provide expertise and education to individuals and groups on health topics.

  • Maintain good working relationship with patients, residents, physicians, FNS staff, visitors and all personnel.

  • Perform clinical nutrition related duties in the absence of the Director, Diabetes & Nutrition Wellness

  • Complies with organization Hand Hygiene Standards.

  • Adheres to organization Standards of Behavior.

  • Performs all other duties assigned.
