Henry Ford Health System Nurse Practitioner Physician Assistant Same Day / Primary Care Woodhaven Full time Day Shift Woodhaven, Michigan
Nurse Practitioner or Physician Assistant will see a mix of Same Day and Collaborative Primary Care appointments. Time will be split between the Same Day Clinic and Primary Care. Schedule is Monday-Friday and 1 weekend morning a month.
Advanced Practice Professional will provide health care and specialized health services to patients and families enrolled in Primary Care Medicine.
The APP will coordinate care of this patient population in collaboration with all members of the health care team.
This position will be 100% outpatient.
APP will provide outpatient medical care by taking patient history, performing physical examinations, orders, and interpreting tests, making diagnoses, and institutes appropriate treatment programs
Independently evaluates patients in the clinic.
Ordering/interpreting testing and lab results, modifying plan of care to address results
Provide clinical expertise and direction to support patient medication management
Maintain accurate documentation regarding patient status, evaluations, and treatments to physicians, streamline the patient transfer process between levels of care, including communicating specific patient needs to internal and external team members and other ambulatory care providers
Perform other related duties as required to assure patient safety and satisfaction.
Participate in the development and presentation of educational experiences for patients, families, and health care professionals
In collaboration with physician(s), APP will provide independent acute and/or routine medical care to a patients and families
Writes appropriate orders, if/when applicable
Practices safety, environmental, and/or infection control methods
Coordinates the activities of patient care and support staff within the clinic
Writes progress notes in EMR indicating patient status and treatment procedures performed
Provides health education to patients, families, and/or community health practitioners and groups
Enhances professional growth and development through participation in educational programs, current literature, in-service meetings, and workshop
May perform clinical research related to specific operations
Performs miscellaneous job-related duties as assigned
Knowledge, Skills, and Abilities:
Knowledge of clinical operations and procedures Knowledge of drugs and their indications, contraindications, dosing, side effects, and proper administration
Ability to maintain quality, safety, and/or infection control standards Ability to perform medical examinations using standard medical procedures
Life support as required through privileging
Ability to clearly communicate medical information to professional practitioners and/or the general public
Ability to educate patients and/or families as to the nature of disease and to provide instruction on proper care and treatment
Knowledge of current and emerging trends in technologies, techniques issues, and approaches in area of expertise
Ability to communicate therapeutically with patients and families
Ability to react calmly and effectively in emergency situations
Knowledge of related accreditation and certification requirements
Ability to make administrative and procedural decisions
Skill in preparing and maintaining patient records
Knowledge of primary care principles and practices
1 year APP experience required.
Must be able to see patients of all ages, and be comfortable supporting Adults undergoing Gender Affirming Care. Experience providing Gender Affirming Care is not required.
GENERAL SUMMARY:
Provides outpatient medical care.
Takes patient history, performs physical examination, orders, and interprets tests, makes diagnoses, and institutes appropriate treatment programs.
Works in collaboration with staff physicians, house officers, nurses, and other medical care team members to ensure that patient outcomes are achieved safely and efficiently.
Specific procedures are individually credentialed as indicated by training and experience.
EDUCATION/EXPERIENCE REQUIRED:
Master's degree in nursing -OR- Bachelor's Degree, completion of an AMA-approved Physician Assistant program.
At least 1 year of APP work experience required.
CERTIFICATIONS/LICENSURES REQUIRED:
Certified Nurse Practitioner. Licensed as a Registered Nurse -OR- State of Michigan Licensure as a Physician Assistant
Current BLS or ACLS as required.
Current ATLS/PALS provider status as required by department.
