POSITION TITLE: Case Manager

DEPARTMENT/PROGRAM: Valley Counseling Services

REPORTS TO:  Clinical Supervisor 

SCHEDULE:  Full time -- 40 hours per week

GENERAL STATEMENT OF DUTIES: * To provide effective community-based coordination and assistance in accessing appropriate services that address the individualized mental health needs of the client. They are directed towards adults, children, adolescents and families and will vary with respect to hours, type and intensity of services, depending on the changing needs of each individual*

ESSENTIAL RESPONSIBILITIES:

  1. Provide specific, measurable, and individualized services to severely mentally disabled or seriously emotionally disturbed client, their families and other individuals.

  2.   Provide services that are focused on the individual's ability to succeed in the community; to identify and access needed services; and to show improvement in school, work and family and integration and contributions within the community.

  3. Provide ongoing assessment of needs and assistance in achieving personal independence in managing basic needs.

  4. Facilitation of further development of daily living skills.

  5. Coordination of the Individual Service Plan.

  6. Symptom monitoring and assistance and coordination in crisis management and stabilization.

  7. Mental health interventions that address symptoms, behaviors, thought processes, etc., that assist an individual in eliminating barriers to seeking or maintaining education and employment.

  8. Conducting activities that increase the individual's capacity to positively impact his/her own environment.

  9. At a minimum, achieve and maintain the Agency's established productivity standards.

  10. Maintain current knowledge of all applicable F&CS policies and procedures and function according to these standards.

  11. Be knowledgeable of and adhere to all pertinent federal, state, and local laws, standards of OMHAS, and appropriate professional ethical standards. 

  12. Comply with COA regulations and educational/certification/registry requirements.

  13. Other duties as assigned.

Requirements

QUALIFICATIONS:

  1.  Ability to establish and maintain effective relationships with clients and significant others.

  2.  Valid driver's license, reliable transportation, and adequate levels of insurance and insurability.

  3.  Must be able to meet agency requirements for criminal back ground check.

MINIMUM EDUCATIONAL/LICENSURE REQUIREMENTS:  Bachelor degree in a mental health related field and a license in social work preferred.

 

*MINIMUM EXPERIENCE REQUIREMENTS: * Prior community support experience preferred.
